To cancel testing/request a refund, please follow the cancellation steps below through your LumiSight account:
- Log in to your account at kidneyhi.workplace.lumisight.com
- Click on 'Get COVID-19 Testing' link
- In the Registration tab, select the registration you wish you cancel
- Click the 'Request Cancellation' button and enter the information requested
- Check your email for a support response
Specify if you are an Oahu resident or not. If you are an Oahu resident, please provide one of the following items for verification and processing by replying to the email:
- a photocopy of your valid Hawaii State ID
- a photocopy of your valid Hawaii State Driver’s License
- a fully executed rental agreement
- a utility bill that shows the refund requester's name on it
*Please note, all refund requests, except for the Oahu resident free testing program, will subject to a $20.00 non-refundable administrative fee that will be deducted from your refund amount and please allow up to seven (7) business days for processing. Your refund will go back to the original form of payment along with an email confirmation/receipt sent to the email used during the initial transaction.
